Annual Declarations are now overdue. Reminders have recently been sent to pharmacists who had a financial interest in a pharmacy business between 1st May 2017 and 30th April 2018 and have not yet completed an Annual Declaration.
If you sold your pharmacy business, exited a pharmacy partnership or closed your pharmacy between 1st May 2017 and 30th April 2018, you must still complete and return the Annual Declaration.
If you have not already done so, complete and return your Annual Declaration IMMEDIATELY. Failure to do so is considered an offence under the Health Practitioner Regulation National Law (NSW) (the National Law).

When completing the Annual Declaration, if you are unsure of the ownership structure in place, search from the Councils’ Register of Pharmacies at www.pharmacycouncil.nsw.gov.au or alternatively check the name displayed at the front of your pharmacy.
If you own the pharmacy in Pharmacists’ Body Corporate (company structure) you must:
- state the name of the Pharmacists’ Body Corporate,
- indicate whether you are a shareholder and/or director, and
- if there is a trust operating in relation to the pharmacy you are required the name of the trust, and indicate if you are a beneficiary or unit holder.
